Medical Social Services provide support and resources to help patients who are facing health issues recover fully. Recovery doesn’t end after hospital discharge. It will take a few weeks or months to recover fully, especially if you have undergone major surgery. In these trying times, our medical social workers can provide compassionate care and emotional support for you and your families. We help you with your health care decisions, provide counseling and education, and help you focus on your recovery.
We will conduct a comprehensive assessment to determine all your support needs. Then, we will work with your physician, family members, support system, and other health service providers to create an individualized discharge plan. This plan should entail your resources for in-home medical equipment, transportation, meal plans, and other follow-up treatments to ensure your fast recovery and optimal well-being.
Here are the social services we provide:
- Community resource coordination
- Assistance with financial aid
- Crisis intervention
- Counseling
- Education on your condition
